We look forward to seeing you for your annual visit! Please take a moment to
read the guidelines below so you’ll be fully prepared for your appointment.
GeneralWhen you arrive, please let us know if your address, phone number or insurance
provider has changed since your last visit. If your insurance has changed, please
bring your most recent insurance card so we can keep the information on file.
Please note that your co-pay is collected when you check out at the end of your exam. Upstate OB/GYN Group accepts cash, personal check or Visa and MasterCard for co-pay payments. HistoryWe’ll begin by taking your health history. Please think about:
A nurse will also record your weight, blood pressure and will take a small blood
sample (finger prick) to send to the lab. Please let us know if you are taking
any new medications
that we are not aware of, including over-the-counter medications and supplements.
Your ExamYour annual exam will most often include a Pap test to screen for cervical or vaginal cancer, a pelvic exam and a breast exam.
Please be sure to let your physician know about any abnormal symptoms you are experiencing, as well as any other health concerns you have. Many patients find that they get the most out of their visits if they prepare a list of questions ahead of time to discuss with their physician. Your physician also may discuss general health guidelines with you, including:
If you are ready for additional screenings like mammograms or bone density screenings,
you physician will write a referral for these services.
Make an AppointmentThe next step is to make an appointment with us. Simply call 864-271-9780, option 1 to schedule an appointment with the physician of your choice. If possible, please
try to schedule your annual check-up when you are not having your menstrual period.
New PatientsIf this will be your first visit with us, welcome! To save time when you arrive,
we invite you to download our new patient packet and complete it prior to your
appointment. Bring your completed packet, along with your insurance card, with
